Below are important forms & documents used to sign up for membership or produce programming here at Lynn Community Television. Click on the document and you will be brought to a PDF of the document. These documents will also be available in printed form at LCTV’s offices.

Required for individual members:

• Individual membership signup sheet
• Cablecast User Agreement
• Copy of ID
• Payment via check made out to “Lynn Community Television”, cash or card on-site, or via PayPal

Required for Sponsor-members:

• Sponsorship membership signup sheet
• Organization Authorization Letter
  • Lists the up to 6 people who will be included under this membership
• Cablecast User Agreement
  • Needed for all who will be using equipment under membership
• Copy of IDs for everyone under membership
• Payment via check made out to “Lynn Community Television”, cash or card on-site, or via PayPal
